Home Health: Registered nurses are available to make a home visit to assess general health status and coordinate a plan care with the clients. Services are provided under the direction of the physicians’ written orders. The nurses may provide instruction about management of short or long term illness, diet, medications and exercise. The may also provide technical care such as dressing changes, injections and catheter care. The nurses assess the individual’s ability to remain at home and can help locate community resources to assist the person to remain safely at home. Specially trained Home Health Aide/Homemakers (HHA/HM) are available to help with bathing, personal care and light housekeeping. The HHA/HM is supervised by a Registered Nurse. Insurance, Medicare, Medical Assistance, other funded waiver programs, and the Veterans Administration or sliding fee may cover costs.

Family Health: Nurses make home visits prenatally to provide education about pregnancy, child development and other topics relevant to pregnant women. A home visit by a nurse is available to all families with a new baby WIC (Women, Infants, and Children) is a supplemental food program for eligible pregnant women, breastfeeding women, infants and children. This program has a strong nutrition education component. Child and Teen Checkup Screenings are available for individuals up to age 21. Screenings help identify medical, emotional and social concerns; referrals are made to appropriate resources. Assistance with obtaining a car seat is also available.

Immunizations: Adult tetanus immunizations and influenza (flu) vaccine immunizations are available as well as the recommended infant and childhood immunizations.
